These Coupon Booklets are available to parents, grandparents, employers or anyone who wants to prepay and save on quality pediatric health services. All services are rendered by the staff at our 365 Day Pediatric Care Centers in Jacksonville, Jacksonville Beach or Fleming Island.

While NOT an insurance policy, 365 Day Kid Care offers employers and employees prepaid pediatric services, saving more than 50% off the cost of normal office visits. These quality preventative services can help protect you and your staff from unexpected leaves of absence due to child health issues.

365 Day KidCare
Fees and Restrictions
- All Coupon Booklets are valid for six months from the date of first use.
- A $10 administrative fee for each weekday office visit is payable at the time of service.
- A $20 administrative fee for each weekend and holiday office visit is payable at the time of service.
- Individual Coupon Booklets are not transferable. Family Coupon Booklets are only valid for family members residing in the same household who are under the age of 18.
- Although we are open 24 hours a day, 7 days a week, your first visit – whether well or sick – must occur between the hours of 8 a.m. and 8 p.m.
SPECIAL NOTE: THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS THE RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF, AND WITHIN 72 HOURS OF, RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, OR TREATMENT.
