At Pediatric Care Centers, we understand that every family is unique, and so are the decisions you make for your child’s health. One of the most personal decisions parents face today is whether to vaccinate. As your child’s pediatric care team, our primary role is to support, educate, and partner with you—whatever your choices may be.
We respect your right to make informed decisions about your child’s healthcare, including the choice not to vaccinate. Our goal is not to pressure, but to provide reliable information and help you understand both the benefits of vaccines and the risks of the diseases they prevent. We believe that open, honest dialogue leads to the best outcomes for your family.

New Vaccine Refusal Documentation
In light of recent updates, the State of Florida and the American Academy of Pediatrics (AAP) have collaborated to create a standardized Vaccine Refusal Acknowledgment Form. This form is designed for pediatric offices across the state to use when a family chooses to decline vaccinations.
This form:
- Reviews each vaccine offered and the diseases they prevent
- Summarizes potential symptoms and complications of those illnesses
- Ensures there is a mutual understanding between the family and the medical provider
While we do not require families to follow a specific vaccine schedule, we are required to document that vaccine education and offerings were provided.
What We Ask
If you choose to decline vaccinations, we kindly ask that you review and sign the Vaccine Refusal Acknowledgment Form. Documenting that we’ve had a conversation and that you’ve been informed.
We’ve attached the form here for your review, and we’re happy to discuss any questions you may have at your next visit.
This is simply a step toward transparency and clarity in your child’s medical record.
